To be promoted, you must successfully diagnose and treat 3 patients.

If you want to be promoted, perform surgery on 3 patients.
Medical Specialist
You must deliver a baby to be promoted.
Surgeon
You will need to successfully diagnose and treat 5 patients to reach the final career level.
Chief Of Staff
This is the final level, so there are no promotions beyond this point.
Can You Access The Hospital Without The Get To Work Expansion?
You can only access the hospital without the expansion if you download a mod. There are numerous different mods available, but only for those playing Sims on PC.
